Diabetes Mellitus Type II has been a failure to treat.

Type II diabetes (Adult Onset Diabetes) counts for over 90% of all cases. The epidemic keeps growing as the number of obese Americans reaches an all time high. We should all be deeply concerned about the onslaught of disease prevalence associated with Type II Diabetes and obesity. They are likely to bankrupt our healthcare system. Drug companies and bariatric surgeons see great PROFITS in diabetes and obesity respectively.

The diagnosis of Diabetes can be established if the fasting plasma glucose is 126 mg/dl or greater on two different occasions. The diagnosis is likely if a patient has specific symptoms, extreme thirst (polydipsia), excessive urination (polyuria), weight loss, overwhelming vaginal yeast infection or yeast balanitis and a random glucose that is over 200 mg/dl. A simple screening can be done by checking the urine glucose via a Diastix which is available over-the-counter. There are infrequently some false positives with Diastix, and patients must have public access so that they can check their plasma glucose with the aid of a readily available professional (Pharmacist) or nurse. If the urine shows glucose, it means that the blood level exceeded the renal threshold of 180 mg/dl and the body dumped excess glucose, a protective mechanism, into the urine. Our immune system does not function well when the glucose is over 180 mg/dl.

For Type I Diabetics (Insulin Dependent) diabetics are thin, young and have excessive eating (polyphagia) and dramatic weight loss and many of the same symptoms described above for Type II Diabetes.

What are the best approaches to reign in Diabetes and Obesity and thus reduce morbidity, and mortality? We must address the various factors that contribute to excess in caloric intake. It may be very difficult for patients with anxiety, stress or depression to follow specific caloric restrictions or guidelines. The provider must meet with the family and encourage certain nutritional changes at home. Instead of investing more on of the same or "Me Too Drugs," we must focus on new horizons.

We must consider expanding the dietitian's role and make them readily available for home visits. Psychologists must be recruited in other to help "bad habits" become "healthy habits." The psychological aspects cannot be underestimated. I do recommend that we begin to prescribe a specific diet for our diabetics, be it through Jenny Craig or new private companies rather than "new" and toxic drugs. We as physicians must be able to prescribe a specific diet or intake per day that is managed by allied healthcare professionals who report to the primary care provider.

For instance, if an adult's daily requirement is 2,000 Calories/24 hours, and we reduce his intake to 1,800 Calories/24 hours, then this patient would lose, on average, 3.8 pounds per month or more if an exercise regimen is added. There is a euphoria associated with weight loss and a dysphoria associated with obesity.

The time has come for us to re-invent our approach to Obesity and Diabetes. The drug companies are poised to develop profitable new drugs. We must view this with fear since many of these drugs are toxic as was Rezulin (troglitazone), which was taken off the market because of liver toxicity. Two similar drugs, Avandia (rosiglitazone) and Actos (pioglitazone), should be studied by the clinician before prescribing them. For instance, Avandia may be less hepatotoxic, but it has potentially serious side effects that include "cardiac events," edema, fluid retention (which may exacerbate hypertension).

Clinicians must encourage their patients to become fiscally responsible, comply with treatment and find ways to modify the environment that has made the consumption of food socially acceptable. The best treatment for Type II Diabetes is through diet and exercise. If drugs are to be used, we already have many generics available, the Sulfonylureas like Glipizide, Glyburide and the Biguanides, Metformin (Glucophage).

It has become evident that our private (and public) companies can no longer afford retiree health care, i.e., General Motors. At present, only 33% of companies with more than 200 employees offer retiree health benefits compared to 66% in 1988. The business community, especially small employers, cannot afford to provide health care coverage as envisioned by Senator Clinton.

"The evolving public morality seems to be turning away from the concept of health care as a right, toward treatment of health as a private matter," NEJM, September 20, 2007. Before we let health care become a private matter, we must educate consumers on how to get the most value out of health care. We must all get involved.

First off what is a calorie?

A calorie is a measurement of certain potential energy in what we eat, we call this food. One calorie can raise the temperature of a single gram of water by one degree Celsius. In a way you can compare calories to gasoline. Gasoline is needed to fuel are cars, and make them "active". Calories fuel our body, and make us "active".

Now that we know what a calorie is, how exactly does are body convert these calories into energy?

The enzymes in are body located in the digestive system break down certain chemical bonds that store food molecules as one. The digestive system then discharges the energy in the bonds, which activates it for use.

What does you body do with the energy?

Like mentioned earlier as gas fuels a car, the calorie fuels everything in your body. It can range from breathing, meditating, growing nails, hair, or running a marathon. When you don't use the calories you consume however, these calories get transported to the liver. The liver restores your glycogen. This is your body's reserve fuel, and is easily accessible. When the liver is full of glycogen, it is then passed to the muscles, for temporary storage. This type of storage will help get you through your exercises. The liver and muscles together carry about 350 calories worth of energy. Think of these calorie storages as accounts, when you consume more, you can save your calories in here until they are converted to fat and spread out through the body.

How many calories do you have to lose in order to lose a full pound of body weight?

The answer is about 3500 calories; you can either eat fewer calories, and burn these off that way, or do some exercises for quicker results. Remember though even if your working out everyday, the calorie rule still applies. If you consume more calories then you burn off, you are going to gain weight. That's the science.

If you want to know how many calories you need to maintain your weight throughout the day, look for a calorie calculator somewhere on the internet; you can also take a BMI test to see what your body mass should be. It has to do a lot with your weight and height.

What is the best way to keep track of your calories?

All you need to do is get a journal, and write down what the calorie amount is for each serving. Write down everything you eat, and add up the calories. If you want to take it into the extreme, you can photograph your meals, and set up some sort of diary or blog about your eating habits. This technique may increase your motivation, and help you understand what you are doing right and wrong. It also gives you a great illustrative view of what your eating throughout the day.

Remember it's all about the calorie count. If you're a male your average calorie amount is about 2,500 calories. If you are a female then your average amount is about 2,000 calories. If you stay somewhere on the line of this calorie count then you will be in the average zone.

Many people want to lose weight but never succeed for various reasons. If you are serious about losing weight, here are some things you need to know.

Do you know that the key to successful weight loss lies in your muscle? Muscles is calorie hungry. A pound of lean muscle requires at least 35 calories each day to maintain itself while a pound of fat tissue needs only 2-3 calories per day. Hence, if you are able to add more lean muscles mass that will require more calories and will tap into the stores of fats you have in your body, you will be able to lose weight, in the form of fat tissue, permanently.

As a result, to embark on a successful weight loss program, you require 3 main components in your weight loss plan. They are:

  • Aerobic Exercises: To burn calories effectively and efficiently
  • Strength Training: To build lean muscles
  • Healthy Diet: To help cleanse and nourish your body
Incorporate 3 these components into your plan for the best result and here are 7 tips to get started
  1. Set Your Goal. How much and by when do you plan to achieve the weight you want. Write them down. You may even want to do a mock photo of how you will eventually look like to motivate yourself.
  2. Make A Plan. No way will just cutting down on what we eat produce any significant weight loss. We need the 3 components mentioned above to work together for maximum impact. So work out an action plan and stick to it. Remember if you fail to plan, you plan to fail.
  3. Put Exercise Sessions Into Your Schedule. Call it Time for Me. Make sure that no one will disturb you during this time.
  4. Eat And Drink Wisely. Start with more fiber. Fiber is good for the body under any circumstances but when we are trying to lose weight, it is even more important. Fruits and vegetables are low in calories and fat and they keep us from being hungry. Simple substitutions are often the easiest way to cut calories. Drinking a diet soda instead of a regular soda every day will save you 15 or more pounds in a year. Eating a turkey sandwich with lettuce, tomato and mustard instead of a roast beef sandwich with mayonnaise just twice a week will save you another 15 pounds. Go through your fridge and pantry and simply throw out all the high calories, high fat snacks and food that are not in your plan. Water is nature's magic elixir. It keeps our organs working with us to lose weight. Eight glasses of water or non-calorie drinks a day is essential.
  5. Learn as You Lose. The more information you have about food and its relationship to weight, the better off you will be and not just while you are losing weight but after the weight is gone. You want to keep those pounds off!
  6. Get Support. Enlist Your Family in Your Battle. Nobody can sabotage a diet faster than a spouse or significant other. Get them on your side from the start. Weight loss is hard at best but it does not have to be lonely. Join a Support group if that can help you.
  7. Chart Your Progress. Keeping a Food Diary will help to get you to keep to the weight you want to have. A progress chart to show you how much weight you have lost will help to motivate you to stay on task.

With these tips and information on hand, they are useless if you do not apply them. So, do not waste any more time. Commit yourself to your goals and plan and you will be on your way to a successful weight loss.
